WHAT WE DO
Hortus Health, situated in the Phoenixville region of Chester County, PA, is a home health agency dedicated to assisting adults with intellectual and developmental differences. Our mission is to offer Pennsylvania state-funded waiver supports, enabling individuals to lead purposeful lives marked by independence and seamless integration into the community. At Hortus Health, we work with individuals whether they choose to live independently, with their families, or in residential communities, helping them acquire essential life skills, forge meaningful social connections, engage actively in the community, and secure rewarding employment opportunities.
