Hortus Health provides quality services through state waiver funding. Hortus Health is approved to provide services under the PA Adult Autism Waiver, ID/A Waivers (PFDS, Community Living, and Consolidated Waivers). We are qualified to provide the following services:
In Home and Community Support
Services typically encompass a range of supports provided to individuals with needs within their homes or in community settings, as opposed to institutional settings. These services may include personal care, assistance with daily living activities, community integration, and other forms of support tailored to the individual's needs.
Community Participation Supports
Support services work with individuals to identify and participate in community-based activities that align with their interests, preferences, and future vocational goals. Community Participation Support Services may encompass a variety of experiences, such as volunteering, recreational activities, educational programs, and other opportunities to foster pre-vocational skills.
Supported Employment
Services are designed to assist individuals in obtaining and maintaining employment in integrated community settings. The goal is to support individuals in securing meaningful and gainful employment opportunities that align with their skills, interests, and goals. Supported Employment services typically involve a range of supports tailored to the individual's needs, including job finding and development, job coaching, workplace accommodations, and assistance with skill development. These services aim to promote independence, enhance vocational skills, and facilitate successful integration into the workforce.
