Emerge Community Services offers a comprehensive range of programs designed to support and strengthen our community.
Emerge Community Services added value comes from real-time mentorship and regular in-person oversight for the entire team from the company CEO, an occupational therapist with more than ten years of experience supporting individuals with autism in Pennsylvania. Emerge Community Services was founded by Heather Buffington, an occupational therapist whose passion is helping individuals find their voice to advocate for autonomy and independence. Each individual served receives support from the clinical team to consult with the individual, family, and staff, to fuel progress towards goals and individualized activities.
Emerge Community Services also benefits from a wide professional network that includes a variety of clinical backgrounds, including Speech and Language Pathologists, Physical Therapists, and Registered Nurses.
Home and Community Habilitation Service
Supports individuals in gaining skills necessary to live within their home and community. Emerge Community Services will help match direct support professionals (DSPs) with consumers to find the just-right challenge, provide scaffolding and develop relationships to help the individual to reach their goals, including activities of daily living (ADLs). Our supports are person-centered and include anything that is important to the consumer in living a more productive, healthy, and independent life.
Community Participation Support
This service is intended to flexibly wrap around or otherwise support community life secondary to employment, as a primary goal. This service involves participation in integrated community settings, in activities that involve persons without disabilities who are not paid or unpaid caregivers.
Supportive Employment
A variety of supports including vocational assessments, job finding, job coaching, and extended supportive employment utilizing a person-centered approach. This approach keeps the individual at the forefront of decision-making regarding their life. Employment not only allows an individual to be included within the community but also feel a sense of purpose and accomplishment.
